Terms of Service

By creating an account or using BreathEasy Forms you agree to these terms. If you are agreeing on behalf of an organization, you confirm you have authority to do so.

BreathEasy Forms is one plan at $49 per month per account. It includes unlimited forms, unlimited submissions, unlimited users, and every feature we ship. There are no per-submission overages, no per-seat charges, and no features held back for a higher tier.

Billing is monthly in advance. Prices for an active account do not increase without at least 30 days notice by email. Payment processing fees charged by your payment provider on forms that take payments are set by that provider, not by us.

The plan is month to month. You may cancel at any time and your account remains active through the end of the paid period. Export your data before cancelling; we keep it available for 30 days after cancellation and then delete it.

Do not use BreathEasy Forms to send unsolicited bulk email, to run phishing or fraud, to collect data you have no right to collect, to host malware, or to break the law. Do not attempt to disrupt the service or access other accounts. We may suspend an account that does these things, and we will tell you why.

You own the forms you build and the submissions you collect. We process that data to run the service on your behalf. We do not sell it, mine it, or use it to train models. Our handling of personal information is described in the Privacy Policy.

We work to keep the service available and back it up continuously, but no online service is perfect. Scheduled maintenance is announced in advance whenever possible.

The service is provided as is, without warranties beyond those that cannot be excluded by law. To the extent the law allows, our total liability for any claim relating to the service is limited to the amount you paid us in the twelve months before the claim.

If we change these terms we update the date above and notify account holders by email. Continuing to use the service after a change means you accept it.
